All newly admitted students are required to submit immunization records. The Immunization Office reviews and manages immunization records for George Mason students.
Here are the questions families frequently ask us:
When do I need to submit my immunization records? What are the requirements?
- All new students for summer/fall 2025 must submit the George Mason Certificate of Immunization form and any supporting documentation by August 1, 2025.
- All required immunizations and instructions are listed on the Certificate of Immunization.
- Follow the 5 steps on the Immunization website to submit the Certificate of Immunization. The Immunization Office highly recommends getting started on this process early as it does take time.
Can I just submit my shot records? What happens if I do not submit the George Mason Certificate of Immunization?
- No, students cannot only submit shot/vaccination records. You must submit the completed George Mason Certificate of Immunization.
- The George Mason Certificate of Immunization must be transcribed (filled out and signed) by a healthcare professional. This can be your healthcare provider or Student Health Services.
What if I do not have a healthcare professional to complete the George Mason Certificate of Immunization?
- Student Health Services can provide transcription service for a fee. For transcription service with Student Health, fill out student sections and sign the transcription consent on the Certificate of Immunization. Upload it, your shot records and immunization documents through the Patient Portal. Visit transcription service webpage.
What are the due dates for fall 2025 enrollment?
- August 1 - Priority Deadline
- September 8 - students whose records are not submitted and complete will have a late fee added to their Student Health Account.
- October 1 - students whose records are not submitted and complete will have a hold placed on their Patriot web account. This hold will affect ability to add and drop classes, and prevent registration for future classes.
